Diablo 4 Season 12 PTR Guide: Killstreaks, Bloodied Gear and New Uniques

Diablo4 Feb-07-2026 PST

Season 12 of Diablo 4, launching February 10th, introduces a fast-paced, combat-focused gameplay loop built around a new Killstreak System and powerful Bloodied Items. This PTR (Public Test Realm) offers a preview of the season’s foundation, with additional features to be revealed closer to launch. The season aligns with the broader roadmap toward the Lord of Hatred expansion on April 28th, where players may look to buy Diablo 4 Gold to expedite their progress.

Diablo 4 Season 12 PTR Guide: Killstreaks, Bloodied Gear and New Uniques

Core Mechanic: The Killstreak System

How It Works

· Trigger: Slay enemies in quick succession to start a Killstreak. A pop-up tracks your count.

· Timer: A short timer begins once the streak starts. If no kill occurs before it depletes, the streak ends.

· Refresh: Extend the timer by getting additional kills, dealing direct damage, or applying damage-over-time effects.

· Tiers: Streaks progress through five intensity levels:

1. Killstreak

2. Carnage

3. Devastation

4. Bloodbath

5. Massacre

Rewards

· Earn Experience and Seasonal Reputation (not active in PTR) when a streak ends.

· Rewards scale with total monsters slain and the highest tier reached.

· Warning: Dying during a streak cancels it without rewards.

New Gear: Bloodied Items

What Are Bloodied Items?

· A new item quality that synergizes with killstreaks.

· Affixes on Bloodied Items grow stronger based on your current killstreak tier.

· Can drop on any item type that normally drops.

Key Bloodied Affixes

· Feast: Example – “Every 25 kills, gain Berserk for 6 seconds. Basic skills gain 70% attack speed.”

· Rampage: Example – “+6% resource cost reduction per killstreak tier when 5+ close enemies are present.”

· Hunger: Example – “10% increased chance for runes during killstreaks. Healing on skill cast, doubled below 50% life.”

New Challenge: Bloodied Sigils

Unlock & Use

· Available after reaching Torment I difficulty.

· Used in Nightmare Dungeons, Infernal Hordes, and Lair Bosses to dramatically increase danger and rewards.

Sigil Effects

· Transforms areas into “bloodied” zones with stronger enemies.

· Nightmare Portals open randomly during combat, spawning dangerous monsters and the relentless Butcher to hunt you.

· Similar to Diablo Immortal’s dynamic dungeon events.
