Diablo 4's Neyrelle's Hand is an important relic in Lord of Hatred Secret Cow Level quest. Before starting, you need access to the expansion's relevant areas and the fishing feature. The route takes you through sin rituals, a ballista mechanism, Amazon training armor, and a crow chase before you obtain a key, fish up a coffin, and enter the temple to retrieve the hand.
If fishing is still unavailable, progress through Lord of Hatred campaign until Fishing introduction opens. Complete it and check that your emote wheel includes the option to cast a fishing line. This hidden quest does not provide ongoing journal updates, so record your selected sins and submitted items as you follow Pride, Anger, Fear, and Greed.
You will temporarily replace your usual Diablo 4 items with an Amazon training set during this quest. Lower the difficulty to something manageable in weaker gear, keep your original equipment in your inventory, and make sure your loot filter shows Common items. You can switch back after burning Mother's Rose.
Find the Outcast's Journal
Travel north from Fool's Quarry waypoint in Skartara and look for a small recess in the mountainside. The red marker shows Outcast's Journal, lying on the ground inside. Its initial message refers to sins, recalling the cedar-tablet ritual at Kyovashad's gates in Diablo IV campaign. Return here after changing sins to check the new text.
Burn Pride and Find the Bolt
Return to Kyovashad's gatehouse, interact with Holy Cedar Tablets, select Pride, and burn the tablet. Revisit the journal before continuing. Its new passage describes war machines and Heavenly Father's spears, pointing you toward ballistas.
This Lord of Hatred clue leads to Akarat's Bulwark in Celestia. Destroy ballista enemies around Observatory Approach and collect Amazonian Bolt for the next mechanism. If it does not appear, check that the journal's text changed and that your loot filter is not hiding the drop.
Open the Hidden Cellar Passage
Head to Eastlooks Bluff on eastern Philios and enter Idyllic Reach cellar. An unloaded ballista stands near the entrance. Interact with it while carrying Amazonian Bolt, then cross the rope created by the shot to reach the hidden passage.
At the end, interact with The Heavenly Father statue and defeat Jailer of Angels. Pick up Father's Chains before leaving the cellar. Take them to Antia's Flame in northern Philios, burn them in Signal Brazier, and record the submission.
Collect Amazon Training Set
Back in Kyovashad, select Anger, burn the tablet, and reread the journal. Visit The Training Grounds in Temis and keep attacking a Training Dummy until Amazon Training Helm drops. Equip it immediately. Diablo 4 has several training areas; this step uses Temis, not Kyovashad's dummy room.
Return to Akarat's Bulwark and defeat Corrupted Amazons. The five-piece sequence is Amazon Training Helm, Training Tunic, Training Pants, Training Gauntlets, and Training Boots. These quest-specific Diablo 4 items must be equipped as you collect them before farming the next piece. If another piece is not dropping, check that you are still wearing everything collected so far.
Collect and Burn Mother's Rose
With all five pieces equipped, look southeast of the observatory entrance for Vasha the Unbroken. Defeat this elite while wearing the complete set and collect Mother's Rose. Regular Corrupted Amazons supply the armor; the rose requires this specific target, so check both the location and the enemy's name.
Take the rose to Temple of Courage in northern Athulua and burn it in Signal Brazier inside. The training set has now served its purpose, and you can put your normal combat equipment back on.
Follow the Crow for the Skull
Burn Fear in Kyovashad and check the journal. Dismiss your mercenary, then travel to the Ruins of Broken Reason in Lycander. Find Large Crow beside the fountain on the ruins' southwestern edge. Avoid attacking enemies during this Lord of Hatred sequence so you do not interrupt the event.
Interact with the crow and follow on horseback. It flies beneath bridges and across lower terrain, so save mount sprints for detours. If you lose it, teleport to Temis and return to the fountain to restart. Complete the loop back to the starting point and collect Hierophant's Skull.
Burn the skull at the brazier outside the Temple of Life on the same island. Father's Chains, Mother's Rose, and Hierophant's Skull must each be submitted at their corresponding braziers. The Hadopelagic Key drops from whichever brazier receives the third item; the submission order is flexible. It is best to use one character for all three burns. If the key is missing, check that this character submitted every item.
Retrieve the Coffin and Neyrelle's Hand
After collecting Hadopelagic Key, select and burn Greed in Kyovashad, then confirm the journal's clue. Go to The Creaking Hulls in Hawezar's Rotspill Delta. Follow the shipwreck to the passage's end and fish from the lighter wooden planks to pull up the Locked Wooden Coffin. Unlock it with the key and interact again to enter the Temple of the Deathspeaker. If you keep catching ordinary fish, adjust your position at the end of the planks.
Inside, reach the altar where you obtained Elias's Finger during Diablo IV campaign. Look nearby for Bloated Calf, which marks the final interaction. Interact with the corpse, collect Neyrelle's Hand, and keep it for crafting.
What Neyrelle's Hand Is Used For?
Neyrelle's Hand is one of three Diablo 4 items required to craft Trophy of the Faithful. Place it in Horadric Cube alongside the base game's Stamina Potion and Vessel of Hatred's Rusted Bardiche, then use the unknown recipe to transmute them. If either of the other relics is missing, keep the hand while completing its separate questline.
Open Trophy of the Faithful for Torn Page, Moo title, and emote. The page directs you to a hidden boat on Philios's eastern shore, leading to Isle of Scylara.
Obtaining the hand completes one relic route; entering Secret Cow Level requires further conditions. The entrance has been solved: the regular route involves Tuesday, and a Moo mechanism provides another way in.
