Step 1: Unlock the Portal to Old Tristram

Before you can even think about the pinnacle boss, you need to reach Old Tristram — complete with its original voice lines and voice actors, a nostalgic treat for longtime fans.

Here's how to get there:

Head to the Imperial Library, where you first met Deckard Cain at the start of the season.

Locate the portal inside the library.

To activate it, you must interact with 10 of Deckard's journals scattered throughout Sanctuary.

Tip: You'll know you're near a journal when a tiny green circle appears on your minimap. If you're missing any, you can always check Deckard's journal to see which part you still need. For example, many players find themselves missing Part 5 while having the other nine.

Once all 10 journals have been collected, a quest marker appears above the Imperial Library. Speak to Deckard Cain, and he'll flip open the portal to Old Tristram.


Step 2: Farm the Three Prime Evil Splinters

To open the big red portal for the boss fight, you need to throw a special item into the fountain in Tristram. That item is crafted from three Prime Evil Splinters:

Splinter of Terror

Splinter of Destruction

Splinter of Hatred

These are arguably the rarest items of the entire season, so prepare for a grind.

Where Do Splinters Drop?

World drops: Extremely rare. You might get one from a Tree of Whispers cache or a seasonal reward chest, but don't count on it.

Main source: Diablo, Baal, and Mephisto in their Torment 10 dungeon versions.

Once you jump from Torment 9 (yellow portal) to Torment 10, you unlock Hell difficulty — the portal turns red. The game recommends clearing Torment 12 before attempting these bosses, and for good reason: Baal, Mephisto, and Diablo hit hard and have massive health pools. You'll want to be dealing tens to hundreds of billions of damage to burst them down efficiently.

Important Myth-Busting

Many players assume:

Terror drops from Diablo

Destruction drops from Baal

Hatred drops from Mephisto

This is wrong. Each of these bosses can drop all three splinters. However, you'll typically need to kill them around 10 times in a row before seeing your first splinter.

Step 3: Why You Should Farm Baal (Not Mephisto or Diablo)

Killing Mephisto 10 times in a row? Not happening. Diablo's run is annoying. But Baal can be reached in about 2 minutes — and the journey there is packed with bonus loot.

Baal Farming Route Breakdown

Entrance trick: If your entrance is at the bottom left and pointing right, the Ariat Summit is always top right or above. If the entrance is on the right, the summit is top left or above. If the connection isn't there, turn around — it's usually right on top.

Ariat Summit: Find the three ancients. Each gives 200 reputation for your seasonal track and drops legendary seals, greater affix items, and gems (Royal Amethyst, Skull, Ruby, Emerald). Stack these gems to craft the big ones for your weapon.

Navigation: After the summit, jump forward to a small area with a connector on top. Jump through, then turn left or right instantly. This area has a few mutations but you'll get a feel for it quickly.

Baal fight: He throws waves of enemies at you. Take them out quickly to reach him faster. After clearing the waves, head through the portal, then jump, jump, and jump over — this saves a huge amount of time compared to going around.

Nuke Baal and pray for a splinter — or at least other great loot like a better Lex Crown or a Mythic Stone of Jordan.

Reset and Repeat

After killing Baal:

Leave the dungeon

Open your map

Reset dungeon

Do it again

It took roughly 10 tries each to get the Splinter of Terror and Splinter of Destruction. The Splinter of Hatred is the current focus. Spamming Lair bosses for splinters doesn't work — even after 20 runs, no luck.

Step 4: Craft the Word's Lucky Coin

Once you have all three splinters:

Take them to the Horadric Cube

Combine them to create an item called Word's Lucky Coin

Bring the coin to the Tristram fountain

Throw it in — the portal to the pinnacle boss fight opens

Make sure you're strong enough for this fight. It's brutal.


Step 5: Why the Hellfire Torch Mythic Charm Is Worth It

The Hellfire Torch is the star of Season 15. Here's what it offers:

Can equip one more unique charm

Yes. Killing Baal again and again is genuinely fun — especially with others, since it's even quicker and each player gets a portal they can share with friends. Fighting Baal beats repetitive Lair boss runs any day. You don't need keys. You don't need anything special. Just be on the right difficulty and rake in the mythics.

Interestingly, Mephisto dropped three Resplendent Sparks multiple times in a row. With 13 sparks banked from a single farming session, you could roll random mythics at the blacksmith (three sparks per roll).
